COVID-19: Vaccine, Booster Clinic Feb 24 At District 5 Area Agency On Aging

COVID-19 Vaccine and Booster clinic will be held at Hawkins Corner

The Ohio District 5 Area Agency on Aging, Inc. is proud to announce a COVID-19 Vaccine and Booster clinic will be held on Thursday, February 24th from 10:00 AM to 12:00 PM, at Hawkins Corner located at 2131 Park Avenue West, Ontario. The Ohio National Guard will administer Moderna First Dose, Moderna Second Dose, or the Moderna Booster to the public.

Anyone 18 and older, and from any county, is welcome to participate in this vaccine clinic. Those interested in participating must make an appointment by calling 567-247-6503. Time slots for the vaccine will range from 10:00 AM to 12:00 PM.

What to bring: Many forms of ID are accepted to verify your name, identity, and age. You do not need to show proof of citizenship or residency status. If you have lost your vaccination card, we can assist you in obtaining a replacement.

Cost: No cost, insurance not required.

The Area Agency on Aging realizes it is critical to ensure equity and access to Ohioans who might not otherwise have a chance to get vaccinated, whether their barrier is transportation, location, mobility, or access to care and education about the vaccine.
