Mass COVID-19 Vaccine Clinic, April 8th, At Hawkins Corner In Ontario

The Ohio District 5 Area Agency on Aging, Inc. in partnership with the Ohio Department of Aging, The Ohio National Guard, and Third Street Family Health Services will launch a mass COVID-19 Vaccine clinic on Thursday, April 8, 2021 at Hawkins Corner located at 2131 Park Avenue West, Ontario.

Anyone 18 and older, and from any county, is welcome to participate in this vaccine clinic. Those interested in participating must make an appointment by calling419-524-4144.Time slots for the vaccine will range from 9 a.m. to 4:00 p.m.

What to bring: Many forms of ID are accepted to verify your name, identity, and age. You do not need to show proof of citizenship or residency status.

Cost: No cost, insurance not required.

The Area Agency on Aging realizes this partnership is critical to ensure equity and access to Ohioans who might not otherwise have a chance to get vaccinated, whether their barrier is transportation, location, mobility, or access to care and education about the vaccine. Ohio has already hosted 103 clinics, bringing vaccines to 5,000 older Ohioans.
