Man Killed Tuesday On Park Ave West Identified

On Tuesday June 11th, 2019 at approximately 4:38PM the Mansfield Division of Police responded to a crash in the area of 452 Park Avenue West involving a 2019 Harley Davidson motorcycle and a 2011 Ford F550 truck.

The operator of the motorcycle, 30 year old Brandon Polak, was pronounced deceased at the scene. He was not wearing a helmet at the time of the crash. A twelve (12) year old male passenger on the motorcycle was transported to a local hospital for treatment of what is believed to be, non-life threatening injuries. The juvenile passenger was wearing a helmet.

The operator of the Ford F550 truck, 23 year old William Mumea and his passenger 18 year old Logan Dauch, were not injured. The crash occurred when the motorcyclist who was traveling westbound on Park Avenue West, struck the rear drivers-side area of the truck which was attempting to complete a left hand turn onto Park Avenue West, into the eastbound lanes from a drive area located on the north side of the street. Both occupants on the motorcycle were ejected at impact.

Mansfield Patrol Officers and a Mansfield Police Crash Reconstructionist at the scene were assisted by the Ohio State Highway Patrol. The crash remains under investigation and no charges have been filed at this time.
